Shrinking Ranks in the Veterans’ Health System: A Crossroads of Care and Policy

On the morning after a long night of service, when a veteran rises early seeking care and finds an empty chair or a sign of delay, we are reminded that health systems are, like communities, defined by the hands that support them. In the coming weeks, a new chapter in that story may unfold at the U.S. Department of Veterans Affairs (VA) — one marked by the elimination of tens of thousands of health care positions across the sprawling federal health care system.

According to internal memos reviewed by The Washington Post and confirmed by multiple sources, VA leadership plans to eliminate up to 35,000 health care roles this month, reducing the agency’s overall health workforce by roughly 10 percent. Most of these cuts will affect unfilled positions — including roles for doctors, nurses, and various support staff — many of which were originally authorized during a period of rapid expansion in the years following the COVID-19 pandemic.

The VA’s health care system is the largest in the United States, serving millions of veterans through nearly 1,400 hospitals, clinics, and community facilities. Officials say the planned reductions will mostly come from positions that have sat vacant for months, if not years, and that the cuts reflect a broader effort to “right-size” the workforce in light of evolving demand and hiring trends. A VA spokesperson told reporters that eliminating open positions will not fundamentally change how care is delivered.

Yet for many inside the department, the announcement has landed with a mix of worry and weariness. Veteran health care staff — including those responsible for scheduling appointments, delivering clinical care, and supporting mental health services — already operate in a system stretched thin by long wait times and recruitment challenges. Union leaders and front-line workers warn that removing authorized positions, regardless of whether they are currently filled, could deepen staffing shortages and increase workloads for the remaining personnel.

The cuts follow a year of significant workforce change at the VA. Earlier reductions — largely effected through retirements, attrition, and earlier reorganization efforts — saw thousands of jobs disappear from the agency’s rolls. Now, these additional eliminations promise to shape the department’s staffing picture well into the new year.

Lawmakers on both sides of the aisle have expressed concern about the impacts on veterans’ care, especially in areas like mental health and primary medical services where even small shifts in staffing can ripple outward into wait times and patient outcomes. Some members of Congress have previously halted or scrutinized sweeping workforce actions at the VA, demanding more oversight and assurances that veterans will not suffer as a result.

From the exam rooms to administrative offices, the practical challenge now facing the VA is how to balance fiscal goals with the department’s mission: providing timely, high-quality health care to those who served. Whether veterans will feel the effects of this workforce contraction — immediately or over time — remains a central question for clinicians, lawmakers, and the veterans themselves. In quiet corridors and waiting rooms alike, the coming months will help answer it.
