“Shadows and Signals: Tracking the New Brunswick Brain Mystery Through Time”

In the spring of 2021, an unusual pattern first caught the attention of health authorities in the Canadian province of New Brunswick. Routine surveillance by Canada’s Creutzfeldt-Jakob Disease Surveillance System (CJDSS) noted a cluster of individuals with neurological symptoms — memory problems, muscle spasm, balance issues and unexplained pain — that resembled rare prion diseases like Creutzfeldt-Jakob disease but repeatedly tested negative for known prion disorders. These signals sparked concern among physicians and families in the Acadian Peninsula and Moncton regions.

As weeks turned to months, dozens of patients were referred to neurologists, many by Dr. Alier Marrero, a neurologist based in Moncton. By late 2021, health officials and media were reporting clusters of 40–50 possible cases with similar, unexplained neurological signs, though there was no confirmed new disease.

In 2022, an oversight committee of neurologists and public health experts concluded — after reviewing these initial 48 cases — that none met criteria for a new neurological syndrome, and the conditions were instead better explained by known diagnoses including Alzheimer’s, Parkinson’s, anxiety disorders, cancer and trauma-related injuries. Health authorities publicly stated no evidence supported the existence of a new disease cluster.

Yet the narrative did not end there. Early in 2023, Dr. Marrero reported an expanded set of patients — numbering in the high hundreds across multiple provinces — suggesting the unexplained symptoms might share common threads and perhaps environmental triggers. Some advocates and medical students cited unusually high levels of herbicides such as glyphosate in patients’ tests and called for deeper investigation.

In 2024, the issue resurfaced politically. With public concern high and families searching for answers, Susan Holt’s Liberal Party promised a fresh, transparent scientific inquiry into the mystery as part of its election campaign a pledge that helped the party win a majority government later that year.

By March 2025, New Brunswick’s chief medical officer of health, Dr. Yves Léger, began a renewed investigation, compiling records from more than 220 reported patients in partnership with the Public Health Agency of Canada to better assess patterns and causes.

In May 2025, researchers from the University Health Network (UHN) in Toronto and New Brunswick published a peer-reviewed study in the Journal of the American Medical Association (JAMA Neurology). This study re-examined a sample of previously labeled “mystery” cases and concluded that all reviewed patients’ symptoms were attributable to well-known neurological conditions, not a novel disease, and found no evidence of a new neurodegenerative syndrome.

January 23, 2026 brought another public update: the provincial investigation’s final report reiterated that no mystery disease was found and recommended follow-up testing and clinical reassessment for individuals involved. Results and data sets will now be forwarded to federal health authorities for a separate scientific review, with no definite timeline yet announced.
