Heavy Steel Beam Falls In Osaka Ward, Three Hospitalized, Ministry Orders Emergency Inspection

Osaka, Japan— A crawler crane dropped a steel beam from height just after 2:30 p.m. on Tuesday at a commercial construction site in Osaka's Nishi ward, injuring three workers on the ground and forcing an immediate evacuation of the structure.

The beam, estimated by site managers at over two tons, slipped from its rigging while being hoisted to the 12th floor. It struck scaffolding on the way down. That impact deflected debris across the lower work zone where a concrete crew was operating. Two men were pinned. A third was hit by a secondary fragment.

Osaka City Fire Department crews arrived within eight minutes. They found one worker conscious but trapped under the beam's edge. Hydraulic jacks and cutting tools were used to lift the load. The rescue took 43 minutes. All three were transported to Osaka University Hospital with what officials described as serious but non-life-threatening injuries.

The site belongs to a mixed-use tower project contracted to a major general contractor based in Kansai. Work had been behind schedule for two weeks due to weather delays. According to a foreman who was on break at the time, the crane had shown intermittent winch issues in the morning shift. The operator reported a sudden loss of tension.

"We heard a loud snap, then the cable went slack," said one worker who asked not to be named because he was not authorized to speak. "People shouted to get clear. There was no time."

The Osaka Labor Standards Inspection Office ordered a full stop-work order for all lifting operations at the site. Inspectors seized the crane's maintenance logs, daily check sheets, and load charts. A preliminary review found the last full third-party inspection was completed three months ago. The daily pre-shift inspection record for today is missing a supervisor's signature.

The Ministry of Health, Labour and Welfare issued a statement late Tuesday confirming it had dispatched a senior safety inspector to Osaka. "We will determine if operational procedures were followed and if equipment was within load limits," a ministry spokesman said. The contractor declined to answer questions at the scene, issuing only a short written apology.

Union representatives for construction workers in Osaka Prefecture gathered outside the site perimeter by evening. They demanded the contractor release all safety audit reports from the past year. One organizer said this is the third crane-related incident in Osaka city this year. Data from the prefectural labor bureau shows lifting accidents remain a leading cause of severe injury.

Police have not filed criminal charges. That decision will wait for the labor bureau's technical report. The site will remain closed at least until the crane is dismantled and tested. Investigators are scheduled to return Wednesday morning with structural engineers.
