In the quiet hours of a hospital room, a mother watches the screen—heartbeat steady, movements reassuring, life blossoming inside. Yet in 2020 and 2021, the arrival of COVID‑19 in the global story cast a new shadow over this tender scene. When the virus crosses into a pregnant woman’s body, it may not simply disturb the mother’s health—it may prompt ripples in the one growing within. Recent research invites us to consider: what happens when the mother’s immune system, stirred by SARS-CoV-2, meets the delicate architecture of a developing brain?

In one large study by Massachusetts General Hospital researchers, which examined more than 18,000 births, babies whose mothers tested positive for SARS-CoV-2 during pregnancy had about twice the odds of a neurodevelopmental diagnosis at 12 months compared to unexposed peers—specifically among male infants. The study noted that at 18 months the increased risk remained but was more modest. The authors emphasised that this does not mean the virus causes autism or developmental disorders in a deterministic way—but an association worthy of attention.

The mechanism may dwell in the mother’s immune response rather than direct viral invasion. When the immune system is activated, inflammatory signals such as cytokines may cross or influence the placental barrier, altering the fetal environment and potentially affecting neural development. For example, one study found altered newborn immune-marker profiles (elevated IL-22, GM-CSF) in those exposed to maternal COVID‐19 during pregnancy. Yet, science is far from a final verdict—the brain’s development is an intricate dance of genes, environment, timing, and chance.

Importantly, other studies offer more reassurance. A cohort published in JAMA Network Open found no significant difference in developmental screening-test scores at 12, 18 or 24 months for children exposed to maternal COVID-19 during pregnancy. Another study at Columbia University Irving Medical Center found that children born during the pandemic (whether or not their mother had COVID-19) did not have higher autism-screening positivity than pre-pandemic children. These contrasting findings signal that—or how much—the risk may depend on infection timing, severity, sex of the child, maternal health conditions, and possibly vaccination status.

What does this mean for expectant mothers and their healthcare providers? First, the absolute risk remains small even if elevated in some groups. Second, vaccination, prompt medical care, and maternal health optimisation (managing comorbidities such as obesity, diabetes, hypertension) remain important—not only to protect the mother but potentially the developing child as well. Third, long-term follow-up matters: early childhood screenings, attentive care, and developmental monitoring may help detect subtle delays sooner rather than later.

We live in a time when the viral wave and the prenatal wave can meet—and the child whose life begins in that intersection asks for understanding, not alarm. The research to date invites caution and curiosity, not panic. It reminds us that pregnancy is not isolated from the broader health ecosystem, and that maternal infection may reverberate beyond the womb—but the story is still unfolding.
