The incident occurred during peak commuting hours when hundreds of vehicles were making the daily journey between the industrial city of Rustavi and the capital, Tbilisi.

According to eyewitnesses and preliminary reports from highway patrol units, the disaster unfolded when a westbound passenger vehicle suddenly lost control, striking a concrete divider. The initial impact triggered a high-speed chain reaction involving several oncoming commuter cars and a crowded transit minibus (marshrutka).
