In a historic first for the International Space Station (ISS), NASA is preparing the first-ever medical evacuation of astronauts from orbit, as the four-person SpaceX Crew-11 mission is set to undock and return to Earth early due to a medical issue affecting one crew member.

Crew-11 — consisting of NASA astronauts Zena Cardman (commander) and Mike Fincke (pilot), Japan’s Kimiya Yui, and Russian cosmonaut Oleg Platonov — launched to the ISS on August 1, 2025 for a six-month expedition originally scheduled to last into mid-February 2026. But a medical concern first identified in early January prompted NASA to reconsider the mission timeline.

Mission managers gave the formal “go” on Jan. 13 for the Crew-11 Dragon spacecraft, Endeavour, to undock from the ISS no earlier than Wednesday, Jan. 14, around 5 p.m. ET, with a splashdown off the coast of California expected the early morning of Jan. 15 (around 3:40 a.m. ET).

The medical issue has not been publicly detailed — NASA is citing medical privacy, and officials have said the affected astronaut is stable. Though the station does have onboard medical resources, the agency determined that more complete evaluation and care on the ground are needed. This decision represents a controlled medical evacuation, not an emergency extraction, and reflects NASA’s emphasis on crew health and safety.

Crew-11’s early departure also included a change of command ceremony, during which Mike Fincke passed control of the ISS to Russian cosmonaut Sergey Kud-Sverchkov — a symbolic and operational shift as the station transitions to a reduced crew while awaiting the next rotation.

Remaining aboard the ISS during this interim are NASA astronaut Christopher Williams and Russian cosmonauts Sergey Mikaev and Sergey Kud-Sverchov, who will maintain station operations until the arrival of Crew-12 — potentially ahead of its previously planned mid-February launch.

This early return marks a milestone in ISS operations: the first medical evacuation in the station’s roughly 25-year history, and a notable test of international cooperation and spacecraft readiness in supporting astronaut health in space beyond routine rotations.
